There appears to be a 1.5mm positive connector on this panel mount 3.5mm, but there is no corresponding 1.5mm connector on the cylinder where the diagram references contact B?

How does one ground this?

Do I simply solder directly onto the cylinder?

Yes you are exactly right. The datasheet shows the B and A termination like you said for the A and B position mating plug.
