This is possibly old HP’s Versatile Link plastic fiber receiver QFBR-2549Z, but obsoleted.

Actually it is hard to find a datasheet. Typically the “4” seems to refer to 30deg tilted package, but
in the photo the package appears to be normal horizontal type.
Wonder if Digi-key has the datasheet in archive?
