What is meant by this voltage rating? Is the maximum operating voltage meant here?
Does this item have a defined range of operation, for example, 12 to 125 VDC?
If yes, will circuit breaker protection be full effective within this changeable range of operation?

Yes, you are correct, the voltage rating is the max voltage the circuit breaker can tolerate before damage will be done to the circuit breaker.
There is no defined range of operations for the voltage as long as you are at or below the circuit breaker voltage rating as the circuit breaker is looking for a surge in the current above its current rating.
