I would like to know if this type of antenna is installed horizontally inside a ground-level junction box under a metal cover.

The antenna is designed to be adhered directly onto a non-metal surface** (e.g., plastic or glass) using tape/adhesive and not intended to be placed flat against metal.
The datasheet recommends keeping it at least 10 mm clear from metal, and ideally 20 mm for best radiation efficiency.
