What are the capacitor values and voltages in this kit KITTYPE1300LF?

KITTYPE1300LF (478-9779-ND) is the KYOCERA AVX Accu-P 0402 thin-film capacitor kit (also listed as 0402KITL1). It contains 30 values with 20 pieces of each. The values and tolerances are shown below:

The kit’s rated voltage range is 16 V to 100 V. The kit documentation (page 9) does not list the voltage rating of each individual value. The Accu-P datasheet shows the voltages offered by capacitance value for the 0402 size.
