Would you have dimensions for the fuse body?
Looks a lot like what Littelfuse calls a “Pico” fuse. Those are about 8mm long, not including the leads.
Here’s a link to the ones we have had in the past. All are obsolete, but one Schurter part, the 7010.6420.13, is still in stock.
Example image:
No way to know if yours is a match for any these or not. Are there any other marks on the non-visible side of your image? I would kind of expect to see an “F”, “S”, or “T” representing fast, slow, or time-lag, respectively.
Waiting on the dimensions. BLF5A are the only markings
3/4" 5amps, 125 volts
Then the item @david listed above would fit nicely.

Also, for option that are still active(not obsolete) the below link of items would be similar to the original fuse you reference. The dimensions are listed on that page to cross check with the full dimensions of the item you have on hand.
5 A Fuses | Electronic Components Distributor DigiKey
Actually, the 7010.6420.13 is significantly smaller than 3/4" (it’s only about 5/16"), so would not be a good option.
