Found a datasheet. Unfortunately what we’ve got on hand for 12V PCB mount relays with 2 form A contacts (here) doesn’t seem to include options with a compatible footprint, but one’s welcome to look for oneself.

It’s a double-pole single-throw (DPST) normally-open 12V coil 5A relay. The trick is the footprint. Your footprint looks like this:
With a pinout of this:
But I can not find any non-obsolete relays with that 15mm spacing between the coil and the first set of contacts. The now obsolete Panasonic ALA2F12 had the correct footprint, though it was only rated for 3A.
