Is there a device - like a small printed circuit board ? - that can do what is illustrated in the included concept sketch ? A variation could be for only 8 connections (and only 3 address bits). The concept is somewhat like 1568-1181-ND (Analog / Digital MUX Breakout) except the important difference is that the switched connections should not be constrained to have any voltage similarity to the address bit voltages.

It is difficult to search for a similar board with capabilities like that since the 1568-1181-ND is an evaluation board based on a specific IC chip.
Depending on how sophisticated or not, you could get an unpopulated perf board and design it yourself. Otherwise, you could try looking at evaluation boards for parts similar to the 1568-1181-ND.

I have not seen anything that meets those specifications. You will probably need to create such a board out of components.
As a starting point, I would take a look at the following part, which, according to the datasheet, has an 8-1 mux with wide supply voltage range of up to 100V and I/O current of 100mA (at 25°C) :
