Need help identifying transistor

I have a transistor with 3 lines of information on it, shown below, any help on identifying manufacture, or type would be a great help. I will ultimately need a replacement but I need to know what I am replacing and its specs. Did a pretty good search and turned up nothing

9504 is almost certainly the EIA date code, 4th week of 1995
