Looking for 12VDC (3/4 TO 1 AMP) power source with stereo jack type end and a 15VDC (.04 AMP) any type end

It’s not entirely clear what is meant by “stereo type jack” here, but if the available power supply options aren’t quite what you need, it’s likely that we have a connector that you could add yourself to a wall adapter that is electrically suitable.
