Greetings,

This would appear to be a custom circuit board assembly, presumably incorporating one of TE’s HTU31 series sensors.

While those sensors themselves are available from DigiKey, a board assembly produced by a third party would not be.

Here’s an Adafruit breakout for the HTU31. It does not have a a filter/cover like the one in the OP so may not work for your application.
