Icharger 16pin ic identity help

I am hoping someone can assist. This is a 16pin ic to drive open the gates for mosfets. Currently it controls 5 mosfets that I can see. When under load, it drops the vcc in from 12 to 6 and outputs 0.5v instead of 12. I have another I was able to confirm with. Without numbers I am at a loss on how to identify. I am a noob and learning. I have been using a multimeter only and soon an ESR meter, but thats all i have access too. Any and all help would be great.

In this scenario it is very difficult to determine what that chip is without either information from the manufacturer or some markings on the part itself.

Sometimes if you list the model of the device that board was from other people online will have more information after having looked for the same thing.

Appreciate the attempt.

11.97v - Pin 1 [ ] Pin 16 - 0v
12.56v - Pin 2 [ ] Pin 15 - 0v
03.29v - Pin 3 [ ] Pin 14 - 12.54v
03.29v - Pin 4 [ ] Pin 13 - 12.54v
04.90v - Pin 5 [ ] Pin 12 - 12.56v
00.00v - Pin 6 [ ] Pin 11 - 0v
12.56v - Pin 7 [ ] Pin 10 - 0v
00.00v - Pin 8 [ ] Pin 09 - 11.97v

The way it is laid out. i know Pins 16, 15, 14, 11, 10 all go to the gate of the mosfets (i beleive it is setup as an H-setup)
